Dr. Jonathan Alvarez is a dedicated general dentist who has created and maintained beautiful smiles for his patients since 2013. He earned his undergraduate degree in Biology from Stetson University in DeLand, FL, before graduating from the University of Iowa College of Dentistry. Originally from Bradenton, Florida, Dr. Alvarez enjoys spending time outdoors with his wife and three children. Whether it's a beach trip or a theme park, he values quality time with his loved ones and creating lifelong memories. Dr. Alvarez is fluent in English and Spanish, allowing him to provide excellent care to patients from diverse backgrounds. He treats all his patients like family, prioritizing their comfort and satisfaction. In addition to general dentistry, Dr. Alvarez is proud to offer specialty services such as Invisalign, simple extractions, pre-molar root canals, anterior root canals, prosthodontics, and sedation using IV or nitrous. Dr. Pantzoulas is well-versed in various dental procedures such as general dentistry, cosmetic dentistry, restorative dentistry, and orthodontics. Dr. Mobeen Alvi, a native of Maryland, attended the University of Maryland, where he earned his Bachelor of Science in Biology and Master of Science in Molecular Biology. In 2015, he went onto receive his Doctorate in Dental Surgery from the University of Maryland, Baltimore College of Dental Surgery, the world’s first dental school. After practicing General & Cosmetic Dentistry for a number of years, he returned to academia to pursue his passion for endodontics. He received his Speciality Certificate in Endodontics from Nova Southeastern University. Dr. Alvi holds memberships in the American Dental Association (ADA), Florida Dental Association (FDA) and the American Association of Endodontists (AAE). Dr. Alvi is married to his beautiful wife of 10 years, and they have two young boys together. Outside of the practice, he enjoys spending time with family and friends, playing golf and basketball and date nights whenever possible!

Dr. Neil De Peralto is a proud graduate of an Ivy League Dental School, the University of Pennsylvania, where he graduated in 2001 with a Doctor of Dental Medicine degree. Driven by the pursuit of offering the most advanced dental treatments, he went on to study orthodontics at Howard University, graduating in 2006 with an orthodontic specialty certificate. Dr. De Peralto’s interest in Orthodontics began when he was an orthodontic patient in high school. This experience fueled his passion for pursuing dentistry. He brings a wealth of knowledge and dedication to Sage and loves creating beautiful smiles for his patients.

Dr. Juan Valencia is a highly skilled periodontist with a passion for providing exceptional dental care. Originally from Cali, Colombia, he moved to Puerto Rico with his family at the age of thirteen. Driven by his dedication to dentistry, he completed his high school education in Puerto Rico and pursued his undergraduate and dental studies at the prestigious University of Puerto Rico. In 2018, Dr. Valencia graduated as a dentist and embarked on a new chapter in Wisconsin, where he participated in the Advanced Education in General Dentistry (AEGD) program at Marquette School of Dentistry. During this intensive program, he honed his skills and gained valuable experience in various aspects of dentistry. After completing the AEGD program, Dr. Valencia worked as a general dentist in Madison, Wisconsin, for a year. However, his passion for providing specialized care led him to pursue a career in periodontology. Following his ambition, he was fortunate enough to be accepted into Marquette's prestigious three-year Periodontics program. Beyond his professional endeavors, Dr. Valencia has a deep love for sports. "Futbol" (soccer) holds a special place in his heart, but he also enjoys basketball and football. In his leisure time, he can be found dancing merengue and salsa, showcasing his vibrant personality and love for Latin culture. During the summer, he delights in grilling Puerto Rican "pinchos" (skewers), creating memorable moments with family and friends. Dr. Juan Valencia is thrilled to be a part of the Sage Dental family, where he can combine his expertise and passion to provide exceptional periodontal care to his patients.
